Job Description

Huruma Aids Concern and Care (HACOCA) is a membership-based, community-focused, non-governmental organization with over 23 years of active experience operating in Tanzania mainland. HACOCA is committed to advancing health, economic strengthening, and education, specifically targeting children, youth, and women. The organization envisions communities free from diseases, ignorance, poverty, and gender-based violence by promoting health and socio-economic welfare through partnerships and collaborations with government and local stakeholders. With expertise in HIV interventions, HACOCA has successfully implemented more than 13 HIV-related projects, addressing critical areas such as HIV prevention education, Testing, STI, TB, PrEP, treatment continuity, Condom use, and viral load suppression.

HACOCA is seeking a qualified Linkage and Case Management Officer (LCMO) to support the implementation of HIV prevention programs targeting populations at risk within the district. The LCMO will work closely with health facilities and the community to ensure the effective navigation of clients to ART initiation, retention in care, and the return to care of those who have missed appointments or been Interrupted in Treatment (IIT). This role will also involve supervising and building the capacity of Peer Navigators and ensuring a client-centered approach throughout service provision.

Key Responsibilities:

  • Coordinate Peer Navigators in communities to support the linkage of KVP-LHIV clients to health facilities for care and treatment services, including monitoring clients on PrEP for adherence and follow-up.
  • Facilitate multiple aspects of client care at health facilities, including appointment management, ART refills, PrEP monitoring, viral load tracking, and the management of clients with Advanced HIV Disease (AHD).
  • Manage cases and report on completed and non-completed referrals for services such as GBV, STI, HTS, PrEP, and FP.
  • Support community-based ART services by overseeing client appointments, medication refills, PrEP adherence, and viral load monitoring.
  • Provide capacity building, mentorship, and guidance to Peer Navigators on PrEP, viral load, and AHD management.
  • Facilitate post-GBV care linkages and track feedback for GBV survivors.
  • Conduct daily follow-ups on case management at each service delivery point, working closely with Biomedical staff, ensuring proper monitoring of PrEP adherence and viral load outcomes.
  • Reconcile and compile data into the regional tracking register and partner listing and outcome register, including updates on PrEP, viral load, and AHD management, ensuring weekly updates.
  • Prepare and submit weekly and monthly reports on tracking services, partner notifications, PrEP refills, viral load, and AHD outcomes.
  • Compile performance reports, site visit reports, presentations, and success stories, including insights on PrEP adherence and viral load monitoring.
  • Document best practices, case studies, and technical briefs on program implementation, particularly around PrEP, viral load monitoring, and AHD management.
  • Ensure proper documentation and record-keeping of client information, including PrEP and viral load tracking, at all supported health facilities.
  • Perform other related duties as assigned by the organization’s management.

Qualifications and Experience:

  • Bachelor’s Degree or Diploma in Nursing, Social Work, Counseling and Psychology, or a related discipline.
  • At least 3 years of demonstrated experience in biomedical HIV prevention, including HIV care and treatment, HIV counseling and testing services, and Sexual Reproductive Health (SRH).
  • Proficiency in spoken and written English.
  • Experience working in HIV prevention programs targeting high-risk and vulnerable populations.
  • Previous work experience in USAID/PEPFAR-supported HIV prevention programs is an added advantage.
